Everyone 3 years of age and older must wear a mask or face covering while in the building.

Only patients with appointments and one 

guardian will be permitted into the office.

We will ask screening questions and measure the temperature of all staff, patients and 

parents.

To limit touch surfaces, we will not supply 

toys, magazines, or coffee during 

appointments.

Leave your phone number with our staff and wait in your car instead of the waiting room.  Staff will call you when it is time to enter the building for your appointment.

Please allow us enough time for enhanced 

disinfections between patient appointments.  Air purification systems are in place.

Please use the provided hand sanitizer when you enter the office.

Social distancing protocols are in effect.  

Please maintain 6 feet of distance at all

times.
